According to the Nursing Home Administrators Licensing and Disciplinary Act, licenses that have expired or been placed on inactive status within the last 5 years may be restored upon payment of renewal fees and proof of successful completion of 36 hours of continuing education. 

However, individuals seeking to restore a license that has been expired or placed on inactive status for more that 5 years will need to file an application with the Department of Financial and Professional Regulation (DFPR) and provide additional documentation that includes, but is not limited to, a current active certification of licensure in another jurisdiction, a statement that the license was in good standing from the appropriate board of licensing authority, or an affidavit. The DFPR may request individuals to submit additional documentation if there are any questions or discrepancies.
